Adult 6V6 Tournament Registration form

By submitting this form, I confirm that I am 18 years of age or older and am registering myself to participate in the Adult 6V6 Beach Volleyball Tournament at Wholesome Pickins Ltd.

I understand that participation in beach volleyball involves inherent risks, including but not limited to: physical contact with other players, falls, impacts from volleyballs, exposure to outdoor elements (such as sun, heat, or uneven terrain), and general physical exertion. I acknowledge that while reasonable safety precautions will be taken, injuries may still occur.

I assume all risks and hazards related to my participation in this league and hereby waive, release, absolve, and agree to hold harmless Wholesome Pickins Ltd., its owners, staff, volunteers, and facilitators from any and all liability or claims resulting from my participation, including travel to and from games or league-related activities.

In the event of an injury or medical emergency, I authorize any necessary medical treatment and accept full responsibility for any resulting costs.

I also consent to receive email communication from Wholesome Pickins Ltd. regarding this league and related activities. I understand I may unsubscribe at any time.

Unless otherwise requested in writing, I grant permission for any photos or videos taken during league play to be used by Wholesome Pickins Ltd. for promotional purposes.
